Admitting Information

For direct admission / with doctor’s order:

1. Admission Types

2. Requirements for Admission

3. Admission Procedure

  1. Registration – Complete the Patient Admission Form at the Admissions Desk.

  • Payment can be made at the Cashier or Billing Counter.

5. Visitor Guidelines

  • Visiting hours: 8:00 AM – 8:00 PM (subject to ward policies).

6. Discharge Procedure

HMO LOA Online Issuance

Approval ng LOA mo, pwede na sa bahay! Hindi na kailangan pumunta at pumila. Sundin lang ang mga hakbang na ito para magamit na ang HMO mo dito sa MTCMC.

How to request medical records

 

Step 1: Complete the Medical Record Request Form

  • Obtain and fill out the Medical Records Request Form available at the Medical Records Unit. Provide complete and accurate information to facilitate processing.

 

Step 2: Prepare the Required Documents

Submit the following documents:

For patients

  • one (1) government issued ID
  • Completed request form

 

For authorized representatives:

  • Completed request form
  • Valid ID of the patient
  • Valid ID of the representative
  • Authorization letter
  • Special Power of Attorney (SPA) if applicable

 

Step 3: Submit your request at the medical records unit

 

Step 4: Payment of applicable fees at the cashier’s unit

  • Once payment is settled, present the sales invoice at the records unit

 

Step 5: Claim the requested records

  • Present the official sales invoice and a government issued ID to the medical records associate when claiming the requested documents. Medical records will be released to the patient or to the authorized representative only.
